IMPORTANT: You are about to complete the Nomination Acceptance Form. This is a very detailed, extensive questionnaire. It will take some time and effort on your part in order to complete. Please ensure that you have both the time and the ability to complete the form. Should your connection be lost or your computer shut down, all of your progress will be lost. Please review the Business Excellence Awards Rules & Regulations where you will also find award descriptions and the criteria on which they will be judged. Please keep this in mind when you are filling in your answers.
By submitting your acceptance for an award(s) you were nominated for, you verify that your and/or your business falls within the criteria for that/those particular award(s). If you were nominated for multiple awards, please only accept those that you want to be considered for. Acceptances discovered not to fall within award criteria will be rejected.
All submissions are final. All acceptances must be received by September 26th, 2016 at 5:00pm.
