You have been recognized by members of the public for your dedication to customer service, business sense and professional spirit. Please accept your nomination by completing the form below if you would like to be considered for an award. Your business will be promoted along with your fellow nominees leading up to and during the Awards Gala. There is no cost to you. You will not be considered for an award if you do not complete the form by the deadline. You may accept any or all categories that you were nominated for. You will find the categories in your acceptance notification email, in the body of the certificate. You will only be considered for award categories that you specifically have been nominated for, qualify for, AND ACCEPT.
IMPORTANT, PLEASE READ: You are about to complete the Nomination Acceptance Form. This is a very detailed, extensive questionnaire. It will take some time and effort on your part in order to complete all of the required fields. Please ensure that you have both the time and the ability to complete the form. Should your internet connection be lost, interrupted or if your computer shuts down, all of your progress will be lost. To maintain security of your personal information; There is no save function.

Please review the Business Excellence Awards Rules & Regulations where you will also find award descriptions and the criteria on which they will be judged. Please keep this in mind when you are filling in your answers.
Written answers are limited to approximately 150 words each. Please stay on topic and be specific with your response. All answers should contain efforts or achievements within the last year. By submitting your acceptance for an award(s) you were nominated for, once you verify that you and/or your business falls within the criteria for that/those particular award(s). If you were nominated for multiple awards, please only accept those that you want to be considered for. While you may accept multiple award category nominations, you may only win one award per award year. All submissions are final. All acceptances must be received by September 27th, 2019 at 5:00pm. PLEASE DO NOT LEAVE YOUR ACCEPTANCE TO THE LAST MINUTE. Required fields will not be accepted without appropriate entries. Forms will be shut off promptly at 5:01pm. If you do not receive a confirmation email, your form has not been received.
